Your documents, unsigned

Please don't sign ahead of time. You'll sign in front of Bob so he can witness it. Have every page ready, filled out except for the signatures.

A valid government photo ID

Each signer needs a current, unexpired ID, such as a driver's license, state ID card, passport or military ID.

Every signer, present

Everyone signing must be there in person and able to understand what they're signing. If you're unsure, especially in a hospital or hospice, call first and Bob will talk it through.
